What to Bring to Your Appointment

Bring your medical insurance card and your vision insurance card, which are usually different, your current glasses, a list of your medications, your sunglasses and, if you wear contact lenses, the boxes.

The essentials

  • Your medical insurance card and your vision insurance card. These are usually two different cards and which applies depends on why you are being seen.
  • Your current glasses, so they can be read on a lensometer and compared against the new prescription.
  • A list of your medications. Antihistamines, antidepressants, blood pressure medication and diuretics all affect the eyes more than people expect.
  • Your sunglasses, so their UV protection can be checked. Not all sunglasses have any.
  • A note of your screen working distance if you spend hours at a computer.

For contact lens wearers

  • The boxes or packaging showing brand, power and base curve.
  • If you have run out and do not know your power, ask your previous office to send it across.
  • Your current lenses if you are updating or reordering.
  • A backup pair of glasses.
  • The bottles of any solution you use for cleaning and soaking.

For a child's appointment

  • Any previous prescriptions, even from another practice. Two or three years of history shows the rate of change immediately.
  • Their current glasses.
  • A note of any concerns raised by a teacher or by a school screening.
  • A rough sense of how much time they spend outdoors on a typical day.

If you are being seen for something specific

For a dry eye evaluation, bring every drop you currently use, including the bottles, and a complete medication list.

For a keratoconus or specialty lens appointment, bring any previous corneal imaging or reports if you have them.

For an Ortho-K consultation, be ready to say honestly how many hours you sleep. Six is the practical minimum for the correction to hold.

What not to bring in a form message

Please do not send detailed medical information, insurance identifiers, payment card details or social security numbers through the website form. Call the office for anything of that kind.
