Privacy Policy

This policy covers information submitted through this website. Protected health information handled in the course of care is covered separately by the practice's HIPAA Notice of Privacy Practices, which is available at the office.

Who this covers

This policy applies to Arizona's Vision Eye Care Center and to information collected through this website. It does not replace the practice's HIPAA Notice of Privacy Practices, which governs protected health information created and held in the course of providing care and is available at the office on request.

What is collected

Information you provide directly through the appointment request form: your name, contact details, preferred appointment times and anything you choose to write in the message field.

Technical information collected automatically where analytics are enabled and you have consented: pages visited, approximate location derived from IP address, device type and referral source.

What is not collected

This website does not ask for and should not be used to transmit detailed medical information, insurance identifiers, payment card details or social security numbers. Please do not include those in a form message. Call the office instead.

How it is used

Appointment request information is used to contact you and arrange your appointment. It is not sold, rented or traded.

Analytics information, where you have consented, is used to understand how the site is used so it can be improved.

How long information is kept

Appointment request records are retained only as long as needed to arrange and follow up your appointment, and then in line with the practice's records retention policy.

Your rights

You may request access to the information held about you, request correction of inaccurate information, or request deletion where retention is not required. Residents of jurisdictions with specific privacy rights, including California and the European Union, retain those rights in full.

Children

This website is not directed at children under 13 and does not knowingly collect information from them. Appointment requests for a child should be submitted by a parent or guardian.

Changes

This policy takes effect on 17 August 2026, version 1.0. Material changes will be reflected here with an updated effective date.
