The notices a patient is entitled to see: how care is provided without discrimination, what language assistance is available, where the HIPAA notice lives, and who is licensed to do what at this practice.

Non discrimination
This practice provides care without discrimination on the basis of race, colour, national origin, age, disability, sex, gender identity or sexual orientation.
If you believe you have been treated unfairly, raise it with the office directly. It will be taken seriously and looked into.
Language assistance
Spanish speaking staff are on site. Dr Nguyen consults in Vietnamese as well as English.
If you need assistance in another language, say so when you book and the office will do what it can to arrange it. Bringing a family member to interpret is common and entirely acceptable, though for anything clinically detailed it is worth asking about proper interpretation.
HIPAA Notice of Privacy Practices
Protected health information created and held in the course of providing care is governed by the practice's Notice of Privacy Practices, available at the office on request.
That notice is a separate document from this website's privacy policy, and it takes precedence in relation to health information. This website's privacy policy covers only what is submitted through the site.
Professional licensing
The doctors at this practice are licensed optometrists, regulated by the Arizona State Board of Optometry.
This practice does not perform refractive or cataract surgery. Where surgery is appropriate, referral is made to an ophthalmic surgeon and post operative care is provided here. Nothing on this site should be read as an offer to perform surgery.
Scope of this website
This site is operated for Arizona's Vision Eye Care Center, at 15215 S 48th St Ste 180, Phoenix, AZ 85044.
There is one location. Any reference anywhere to multiple offices is incorrect. There is one telephone number, (480) 706-3937, and one contact address, service@arizonasvision.com.
Appointment requests
Requests submitted through this website are requests, not confirmed bookings. The office follows up to confirm a time.
Do not use the website form to report an urgent symptom. Call the office during opening hours, or go to an emergency department outside them.
